Build a neutral chronology
- Date-stamped imaging reports and key procedure notes.
- Pathology or biopsy summaries if relevant — full PDF, not only SMS.
- Prior written treatment plans or discharge summaries from each facility.
What to leave out or separate
Chat screenshots and long narrative threads are hard to verify. Prefer official PDFs; keep a short personal diary of symptoms in a separate document if your doctor asked for it.
