Three layers to pack
- Identity and admin: photo ID, insurance card or policy number, referral letter if the hospital issued one.
- Clinical context: recent lab PDFs (dated filenames), discharge summaries if the visit relates to a prior admission, current medication list with doses.
- Your questions: top three concerns on paper or in HealthArc notes so nothing is forgotten when the clock is short.
Timing and copies
Upload or scan reports as soon as you receive them; do not rely on hospital portals alone on the morning of the visit. Keep one "master" PDF per investigation date to avoid duplicate pages in the same folder.
